    Positions Details

    What does our Resident Care Coordinator do?

    • Actively supports the nursing and care staff where assistance is needed.
    • Coordinates arrangements for resident appointments, treatments, or procedures, including transportation
    • Supervises and observes staff in the performance and their duties; provides them with feedback and any needed guidance
    • Maintains open communications daily with the Resident Care Manager on the status of all residents and staff.
    • Assists the Resident Care Manager in providing proper orientation and training, timely performance reviews, and counseling/discipline on performance concerns.
    • Coordinates follow through of the medical records process and validates compliance.
    • Works as assigned by the Resident Care Manager on issues related to quality of care through completion of assigned reports and assessments in care related areas such as infections, psychotropic’ s, skin issues, weight reports and behavior issues.
    • Complete ULP education as delegated by Director of Nursing to meet State of Minnesota compliance.
    • Integrates and supports the assessment, planning, and implementation of the admission, transfer and discharge process.
    • Participates in investigating, documenting, and resolving complaints by residents and family members.
    • Other duties as assigned or may be necessary in an organization having overlapping roles and responsibilities.


    What we're looking for from you:

    • Education and Training: Four or more years’ experience in a long term care setting as a licensed nurse. Supervisory experience preferred.
    • Knowledgeable: State regulations pertaining to comprehensive home care license. Point Click Care software knowledge a plus
    • Leadership: Serve as a role model and provides leadership in problem solving among nursing and care staff. Creates and promotes an atmosphere of respect, personal integrity, and teamwork.
    • Licensure and Certifications: Currently licensed in the State of Minnesota as a LPN, with license in good standing
    • Superb Communication: Must be able to read and write sufficiently in order to complete responsibilities of the position
    • Problem Solving: High level of problem solving and conflict resolution capabilities are necessary to this position
    • Excellent Attitude: Have fun and laugh while taking pride in your work environment
