    Positions Details

    MDS RNs are critical members of the VCC care team! Here are some the essential items you'll be doing in this important role:

    • Manage the resident assessment information of the Minimum Data Set (MDS) forms, ensuring they are completed, reviewed for completeness and accuracy and processed in a timely manner
    • Communicate with families and insurance companies to coordinate levels of care and therapy
    • Attend care planning conferences
    • Assist in training new team members
    • Ensure follow-through on dietary and pharmacy reviews and incident reports
    • Ensure medications and supplies are ordered, in stock and counted
    • Assist the DNS in the initial admission process to evaluate clinical and insurance information of potential residents
    • Assure all charting and record keeping is accurate and complete
    • Integrate and support the assessment, planning and implementation of the admission, transfer and discharge process of residents
    • Promote the Mission, Vision and Values of Transforming Age in all areas of responsibility

    Our ideal candidate will:


    • Be currently licensed in the State of Washington as a RN with license in good standing
    • Have two or more years’ experience in a long term care setting as a licensed nurse with supervisory experience preferred.
    • Have knowledge of State and Federal regulations pertaining to long term care
    • Have knowledge of minumum data set (MDS) process and experience with Medicare requirements
    • Have knowledge of DSHS Regulations
    • Possess superb interpersonal and communication skills and the ability to work well and collaboratively with team members and external constituents
