    We are seeking a full-time Social Services Coordinator to join our team. The schedule is Monday thru Friday, day shift.

    Positions Details

    The position is responsible for identifying the social, psychological, and emotional needs of residents, and for making arrangements for services to meet such needs such as assisting Parkshore residents’ make a successful adjustment to their condition and to the community.

      

    Principle Responsibilities

    • Works with all available resources to arrange for services to meet residents’ needs and facilitate their adjustment.
    • Makes routine contacts with staff and/or residents of Health Center, and the Assisted Living as needed.
    • Participates in resident admission and discharge and provides necessary documentation, as needed.  Assists with room assignment, identifies potential behavior issues, identifies possible barriers to discharge and develops interventions for each.
    • Interviews and writes social history, assessments, and care plans for incoming residents and follows-up with regularly scheduled meetings and reviews.
    • Coordinates transfer of resident from one level of care to another.  Actively participates in transitional care meetings.
    • Completes and keeps current S.S. portion of MDS and relative RAPs, Care Plan, quarterly/annual reviews.
    • Documents and charts care plans, assessments, results, meetings, and changes.
    • Meets and counsels with residents and families as needed.  Provides in-service training to staff and residents.
    • Performs necessary administrative functions, record keeping, correspondence, and communications.
    • Maintains the Resident Handbook for HC and AL and provides specialized handouts as needed.
    • Maintains policies and procedures for the Social Services department.
    • Completes Medicare decertification paperwork and documentation, including documentation of resident/family notification.
    • Develops and maintains community resources and contacts.
    • Works cooperatively with government regulatory agencies.
    • Assists residents with Medicaid/Medicare concerns.  Participates in weekly Medicare review meeting and updates resident and family as needed.
    • Keeps log of residents with orders for psychoactive medication.  Addresses diagnosis and target behaviors.  Partners with nursing and other professionals regarding interventions including routine dose reduction trials to help manage the behaviors using the lowest dose of psychoactive medication possible.
    • Keeps educational credentials and professional certificates current.
    • Develops goals and objectives annually.
    • Develops and maintains outside professional contacts.
    • Performs other duties as assigned.
    • Promote the Mission, Vision, and Values of Parkshore in all areas of responsibility.
    • Other duties as assigned or may be necessary in an organization having overlapping roles and responsibilities.

    Required Skills/Knowledge/Abilities

    • Experience in Geriatrics, Medicaid, Medicare, and 3rd party reimbursement is required. 
    • Must possess excellent communication, leadership, and organizational skills, and have a high degree of flexibility and adaptability. 
    • Must work well as a member of a team and be encouraging and supportive. 
    • Must have excellent interpersonal and communication skill.

    Education and/Equivalent Experience

    • Masters in Social Work is preferred. 
    • Minimum requirement is a four year college or university education with a B.A. /B.S. degree in group work, psychology, counseling, social work, or related field.
