    Positions Details

    As a critical member of the Skilled Nursing team at VCC, our CNAs are responsible for the following:


    • Providing residents, families and team members with exceptional customer service at all times.
    • Communicating in a respectful, professional and team-oriented fashion at all times.
    • Promoting and advocating for resident rights and safety at all times.
    • Maintaining confidentiality of resident information.
    • Reporting and preventing neglect and abuse and situations.
    • Acting as an integral part of the care team, reamining knowledgeable of each residents’ history, health status, strengths, needs and personal preferences.
    • Regularly reporting changes in resident condition to licensed nurse or direct care staff in a consistent and timely manner.
    • Assisting resident and interdisciplinary team in development and implementation of plan of care.
    • Attending and participating in care conferences.
    • Providing holistic, consistent, and compassionate care and assistance to residents with personal hygiene and grooming needs.
    • Assisting in meal preparation, personal laundry, and housekeeping as needed.
    • Working with residents to develop scheduling of daily care based on the resident preferences, past history/lifestyle.
    • Under the direction of a licenses nurse, monitoring and documenting vital signs
    • Determining the resident’s strengths and positive attributes, helping build on those strengths to optimize quality of life.
    • Encouraging and assisting resident with restorative and rehab care, determining ways, through activities of daily living, to assist resident to remain as independent as possible.
    • Assists residents in meeting emotional and social needs.
    • Attending social events with residents.
    • Establishing meaningful relationships with residents' family members.
    • Actively promoting and assisting residents and their families in making VCC feel like home.
    • Understanding and following fire safety, disaster preparedness and emergency care policies and procedures.
    • Attending in-services and actively participating in staff meetings, change of shift reports, and committee meetings.
    • Promoting the Mission, Vision and Values of Transforming Age in all areas of responsibility.


    Education and/or Experience, Knowledge and Skills


    • Possess a Nursing Assistant Certification
    • Experience with older adults or desire to serve older adults in a caregiving role; Skilled Nursing Community-based experience is highly preferred.
    • Excellent communication skills, both written and verbal, in the English language.
    • Highly motivated, organized and disciplined to work without direct supervision.
    • Able to make independent judgment decisions.
    • Have a caring, passionate personality and ability to develop and maintain meaningful, positive relationships with residents, patients, families, physicians and team members.
